Nursing Home Admissions
A survey/assessment is done to qualify someone based on need prior to them being admitted into a skilled nursing facility (nursing home, rehab) is know as a PAS (Pre-admission Screening). If they have come directly from the hospital then it is executed there. If they are coming from home it must be completed and approved prior to admission which usually takes a minimum of 48 hours. Potential patient must have -
A recent History & Physical from their physician
New prescriptions/orders for all their medication
A current (within thirty days) chest x-rayT.B. test and the results
Completed and sign Application for Long Term Care
Advance Directives
They must meet the need for 24 hour care by not being able to perform at least two Assistance of Daily Living (ADL's). They can also qualify based on safety and the need to cue them, often in the case of a dementia patient.
